The hairline is one of the most recognizable features of a face, and for people pursuing gender-affirming care it can matter as much as any other step in a treatment plan. Pacific Northwest Hair Restoration provides surgical scalp hair restoration as part of that care, performed by Dr. Andy Higgins at our Tigard office in the Portland area. This page explains what the procedure involves and how coverage works for Oregon Health Plan members through PacificSource Community Solutions.
This page covers:
Scalp Hair Restoration in Gender-Affirming Care
Hair transplantation moves follicular units, the naturally occurring groupings of one to three follicles that grow together, from a donor area on the back and/or sides of the scalp to a recipient site. The transplanted follicles retain the growth characteristics of the donor area, which is why the technique is used to build density in places where hair does not currently grow.
Pacific Northwest Hair Restoration performs both follicular unit extraction (FUE), in which follicles are harvested individually, and linear strip excision (LSE), also called FUT, in which a strip of donor tissue is removed and dissected into grafts. Robot-assisted FUE using the ARTAS iX system is also available. Which approach fits a given case depends on donor supply, the size of the area being treated, and the density the plan calls for.
Hairline Advancement and Temporal Restoration
A hairline shaped by androgen exposure typically sits higher at the temples, with a recession pattern that creates an M-shaped frontal edge. Hairline restoration in a feminizing context involves lowering the frontal hairline and rebuilding the temporal points to create a rounder, more continuous frontal outline.
This is a design problem before it is a surgical one. Dr. Higgins plans hairline position relative to facial proportions, assesses the density available in the donor area, and determines whether the intended outline can be achieved in a single session or is better staged across two.
Some patients pursue hairline restoration on its own. Others coordinate it with facial surgery performed elsewhere, in which case the sequencing of procedures is worth raising early, because hairline design and other facial work can affect each other.

Coverage Through PacificSource Community Solutions
Gender-affirming health care is protected under Oregon law. Since January 1, 2024, Oregon insurers cannot deny or limit coverage for gender-affirming treatment a provider determines is medically necessary under accepted standards of care. Oregon law also requires coordinated care organizations to maintain access to gender-affirming treatment providers without unreasonable delay.
PacificSource Community Solutions is one of the coordinated care organizations that administers the Oregon Health Plan, Oregon’s Medicaid program. Pacific Northwest Hair Restoration has an agreement with PacificSource Community Solutions to provide gender-affirming scalp hair care to its members and is reimbursed at a contracted rate.
Two things are important to understand about that agreement. It is approved case by case, meaning each patient is authorized individually rather than under a blanket benefit. And it covers scalp hair restoration only. Facial hair restoration is not part of the current agreement.
How the Authorization Process Works
The sequence is the same for every case:
Because approval is individual, no one can tell you in advance that a specific procedure will be covered. What the team can do is start the authorization process and tell you honestly where a case stands. If you are an Oregon Health Plan member, the clearest first step is a conversation with your primary care provider or your coordinated care organization care team, who can begin the referral.
If You Are Not an Oregon Health Plan Member
Commercial insurance plans in Oregon are also required to cover medically necessary gender-affirming treatment, though documentation requirements and prior authorization processes vary by carrier. Bring your plan information to the consultation and the team will review what your plan requires.
Hair restoration performed for reasons other than gender-affirming care is not covered by insurance, and financing through Cherry, PatientFi, and CareCredit is available for those procedures.
Surgical Care with Dr. Andy Higgins
All surgical procedures at Pacific Northwest Hair Restoration are performed by Dr. Andy Higgins, a board-certified general surgeon with more than twenty years in practice. His surgical background spans trauma, vascular, thoracic, laparoscopic, endocrine, pediatric, and colorectal surgery. He now focuses his practice on hair restoration.
Consultations are physician-led. Dr. Higgins evaluates donor supply, examines the recipient area, discusses what a realistic graft count can accomplish, and explains where the limits are. If transplantation is not the right step, or not the right step yet, he will say so.
Where Procedures Are Performed

Tigard
All hair transplant procedures, including ARTAS robot-assisted FUE, are performed at the Tigard office in the Portland area.

Bend
The Bend office provides consultations, follow-up visits, and PRP treatments, which means patients in Central Oregon can complete much of their care close to home and travel to Tigard for the procedure itself.
What to Expect
Transplanted follicles shed within the first several weeks. This is expected and is not a sign the procedure failed; the follicle remains in place and re-enters a growth cycle. Meaningful density typically becomes visible between six and nine months, with results continuing to mature through twelve to eighteen months.
Recovery involves a period of visible healing. Small crusts form at the graft sites and resolve over roughly one to two weeks, and the donor area requires care during that time. Individual results vary based on donor density, hair characteristics, the size of the area being treated, and how the transplanted hair is maintained afterward. Post-operative care runs for a full year after the procedure.
The Consultation
The consultation is an evaluation, not a sales appointment. Dr. Higgins and the team will review your goals, examine the donor and recipient areas, discuss graft counts and staging, and outline what a treatment plan would involve. If coverage is part of the picture, bring your plan details and the team will walk through what the authorization process looks like for your situation.
Frequently Asked Questions
Is hormone therapy required before a hair transplant?
Not necessarily for hairline work. Oregon Health Plan surgical criteria may reference hormone therapy duration, so what applies in a given case depends on the plan requirements for prior authorization and on your care team. Dr. Higgins will discuss timing at the consultation.
Does PacificSource Community Solutions cover this?
Gender-affirming hair care is a covered benefit, and Pacific Northwest Hair Restoration has an agreement with PacificSource Community Solutions to provide it. Approval is granted case by case, so each patient is authorized individually, and a contract is signed for that specific case. No one can confirm coverage for a particular procedure before that authorization is complete.
Does the coverage agreement include beard or eyebrow restoration?
No. The current agreement covers scalp hair restoration only.
Where would my procedure take place?
At the Tigard office in the Portland area. The Bend office handles consultations, follow-up visits, and PRP treatments.
How many sessions does hairline restoration take?
It depends on how far the hairline is being advanced, the density required, and the donor supply available. Some plans are completed in a single session and others are staged. This is determined at the consultation rather than in advance.
How long does transplanted hair last?
Transplanted follicles keep the growth characteristics of the donor area they came from, which is why donor-area selection matters. Longevity varies by individual, and hair in surrounding untreated areas can continue to change over time. Dr. Higgins will discuss what that means for long-term planning.
